If you do not want to order your AMS membership via the Web, print a copy of the membership application, fill it in, and proceed with the payment method of your choice.

By fax: Print a copy of the 2012 application, fill it in, and fax it to the attention of AMS Member & Customer Services at 401-455-4046. Please include credit card information or request a pro forma, as the AMS requires prepayment.

By mail: Print a copy of the 2012 application, fill it in, and send it, along with a check, credit card information, money order, or UNESCO coupons to: American Mathematical Society, P.O. Box 845904, Boston, MA 02284-5904, USA. Payment by check must be made in U.S. currency and drawn on a U.S. bank.
