AMS Coversheet

The AMS Coversheet is a paper form which should be added to all job applications made to academic Departments of Mathematics. The paper form is available in several formats. Choose the appropriate format based on your computer resources. If none of the electronic formats are appropriate, use the form printed in each issue of the Notices of the AMS. If needed, you can access the list of two-digit Mathematical Subject Classification codes.

1. Cover Sheet - TeX Template. TeX is a mathematical typesetting software. This form will ftp to your computer. Choose this ONLY if you are an experienced TeX user and have the software available. You will be prompted for the text of each answer field as the document is TeXing. There are instructions included for re-using the file containing your answers, if you need to edit them later. Read all instructions given in the form carefully.
2. Cover sheet - PDF version. This will download a PDF file to your computer from which you can print out a blank form. PDF files can be opened, after downloading, using Adobe Acrobat Reader (free, downloadable software).
3. Cover Sheet - Word for Mac 6.0 Template. Form contains text fields which may be filled in using Word, and then printed. May be used on Macintosh computers with Microsoft Word software. Document is protected from changes after downloading to ensure that it prints in the correct format. The answer fields may be edited at any time.
4. Cover Sheet - Word for Mac 6.0 Printable Blank Form. Download this version if you only want a blank version of the form to print or type. The Job Application Database for Mathematicians (Mathjobs.org)

The AMS is now sponsoring a high-quality database system (originally developed at Duke University) which offers a paperless application process to applicants and employers in mathematics, at www.mathjobs.org. Those hiring for positions are encouraged to use the system, which is offered at an annual fee of US$400. There is no charge to applicants. Applicants, upon registering, can make printable AMS coversheets and create their own portfolio of application documents. They make job applications online to the participating employers. Employers, after registering, can choose among many electronic functions, and customizable features. Letter of reference can be sent by letter writers for the jobs being applied for.
